Thank you for making your wellbeing appointment with Consciously Well.  Prior to the consultation we require you to complete our confidential Pre Consultation Health Questionnaire.  This enables us to have a complete picture of your requirements prior to seeing you.

The survey usually takes about 30 minutes, please take your time.  This sets the foundation for a more effective and complete initial consultation.

Please note: we recommend completing the survey in one sitting for continuity, however if that’s not possible, it will automatically save at the end of each page and you re-access the survey by clicking on the button below. (You will need to be using the same computer for this to be possible)
